Integrated Care Services

At Liberty Community Programs, we provide compassionate, person-centered care in a peaceful, home-like setting. Our integrated services support adults and adolescents facing mental health challenges, substance use disorders, and developmental differences.

Each program combines professional clinical care, residential support, vocational training, and community engagement to help individuals heal, grow, and reconnect. Residential & Group Home Services

Our residential and group home programs provide safe, structured environments where individuals receive the support they need to live with dignity and independence.

Specialized group homes serve individuals with intellectual and developmental disabilities, including autism, cerebral palsy, and traumatic brain injuries. Residents receive 24/7 support with daily living skills such as personal hygiene, dressing, meal preparation, medication routines, and using assistive devices.

Our trained teams include behavior support professionals who provide crisis intervention and individualized care. Additional services may include nursing support and therapies such as physical, occupational, and speech therapy when needed.

Mental Health & Behavioral Services

We provide comprehensive mental health care for individuals and families navigating emotional, behavioral, or psychiatric challenges.
